Specialised housingfor adults under guardianship in La Rioja

Fundación Salud y Comunidad (FSC), in consortium with Grupo Lagunduz, manages the Specialised housing for adults under guardianship in La Rioja, intended for the comprehensive care of adults diagnosed with a mental disorder who are subject to judicial proceedings for the modification of their legal capacity, and whose guardianship has been entrusted by the courts to the Fundación Tutelar de La Rioja.

The service has two homes with 14 places in the city of Logroño, plus one emergency place that is used in urgent situations or in cases of duly accredited need.

The professional team of these supervised flats is made up of two educators (one of whom also acts as coordinator) and eight educational assistants, who follow an intervention pathway and a specific Individual Care Plan model for each service user.

The general objective of the service is to provide these people with a residential resource that facilitates comprehensive enabling care, with the aim of keeping them cognitively, physically and functionally active, re-educating their impaired faculties, ensuring that they follow their medical treatment and, ultimately, improving their quality of life.

The Programme is framed within an intervention model that integrates in practice several cross-cutting methodological approaches: Person-Centred Care (ACP) and the Quality of Life model, which are complemented by other core components of our care model, such as Positive Behaviour Support and the Supports Model, based on the promotion of the rights of service users with disabilities as the core of the model:
